About this Service
The mission of the Primary Care Integration Program is to ensure access and coordination to primary health care services for those accessing services at the Clara Martin Center. Clara Martin Center has worked to develop strong, collaborative relationships with primary care practices within our service area.
Clara Martin Center is co-located with Gifford Health Center in Chelsea at the Chelsea Health Center and works closely with their offices in Randolph, Bethel, and Rochester.
Clara Martin Center also provides contracted Care Coordination services at the White River Family Practice in White River Junction. Other primary care offices in which there is a strong relationship include Little Rivers Health Center – a federally qualified health center (FQHC) with offices in Bradford, Wells River and East Corinth; Upper Valley Pediatrics in Bradford; Newbury Health Clinic in Newbury; and Ammonoosuc Health Center in Woodsville, New Hampshire.

Clara Martin Center Hosts 2022 Annual Meeting and Awards Ceremony
The 2022 Annual Meeting and Awards Ceremony for Clara Martin Center was held on the Vermont Technical College campus on Thursday, November 9th to honor employees for their dedicated years of service.

Clara Martin Center Awarded $4 Million SAMHSA Grant to Continue Certified Community Behavioral Health Clinic Expansion
Clara Martin Center is the first community mental health agency in Vermont to have been designated a CCBHC after receiving an initial two-year CCBHC Expansion grant from SAMHSA in 2021.
